Off Campus Submissions:

If submitting application through the Blackboard drop box, it is the student’s responsibility to email documents that require signatures to send to the appropriate offices on campus.

On Campus Submissions

• Can use drop box OR
• Drop the application in my mailbox outside the practicum instructor’s office door

Advisement clearance:

• Leave blank as the practicum instructor will sign document upon review of Degreeworks.

Site Supervisors

• Make sure to include a secondary site supervisor signature for those completing an internship.
• You must have two supervisors. Applications will not be approved unless you have two supervisors
